An applicant for the $500 book grant must meet the following requirements:

      1. Resident of Henry County, Ohio seeking degree in education.

      2. Be in the upper third of the class scholastically.

      3. Shows evidence of leadership potential through activities, some of which
          should be academic and community service.

      4. Submits the following before the postmarked deadline of April 1:

• Complete the Application Form from the downloadable form available on this website: Name, Address, Land Phone, Cell Phone, E-Mail, High School, Graduation Year / GPA, College, Year(s) in College, Major and Minor, Signature and date.
• A current transcript
• Three (3) letters of recommendation from: 
•  A Teacher
•  A Community service organization advisor
•  Another unrelated adult who has no connections to your school
•  Complete the High School Activity Checklist to list church, school, and community involvement in the last 4 years, including years of participation and leadership roles and offices held.  Use link on the website to access this checklist.
•  A one page essay answering the question: What strengths do you possess that will make you a great teacher.
